Consultmed, Personify Care partner to deliver end-to-end patient pathway.

Consultmed is excited to announce our partnership with Personify Care, delivering a much-needed, end-to-end patient pathway from the first GP visit and referral to specialist services, triage, admission, consent and enrolment into a digital patient pathway.

This collaborative approach and our integration will be launched later this year at Wandi Nerida, a specialist residential eating disorder recovery centre in Queensland and will be made available to other health providers managing elective surgery and outpatient services.

Here is how it will work at Wandi Nerida:

  1. GPs and other primary care providers can send an electronic referral directly to the specialists or the hospital using Consultmed’s web-portal or directly from their practice management software (e.g. Best Practice, Medical Director)
  2. Psychiatrists and Eating Disorder clinicians will be able to use Consultmed to triage the referrals and provide specialist Advice and guidance to referrers. A Case Conference can also be initiated between treating clinicians to discuss patient care plans using our secure messaging technology, built on MS Azure.
  3. Once the referral is accepted, scheduled or waitlisted, the patient is automatically enrolled into a personalised Digital Patient Pathway via our integration with Personify Care.
  4. The clinical team will track the admission and preparation process via Personify Care’s Digital Patient Pathways, preparing patients for their first appointment or admission, obtaining consent, and collecting patient reported outcome measures (PROMs).
